Common questions about this example

Do I have to list my former spouse if we divorced years ago?

Yes. When your marital status is Divorced, the form asks about each former spouse, however long ago the marriage ended: their name, date of birth, nationality and place of birth, and when, how and where the marriage ended.

Is my married surname another name?

The form asks for every other name you have used, a maiden name included. Jovana used her husband's surname while she was married, so she lists it even though she no longer uses it.

What do I write for how the marriage ended?

A short, plain statement in English, such as a divorce or an annulment and the court that granted it. The box rejects some characters, such as the slash, the colon and quotation marks.
